Step 1 - Get a Referral from your doctor

We require a referral from your physician or advanced practice provider. Please have them fax the referral to 903-593-4303.

Dr. Jetter will review the referral and accept you as a patient if she feels she is able to help you. If you have seen a neurologist in the past, please have these records sent to our office.

step 2 - complete and return your paperwork

After you are accepted as a patient, we require the following paperwork to be filled out completely and returned to us. Please fill out the correct paperwork below based on the problems you are having:

Fill out this paperwork if being seen for seizures, spells or blackouts.

Fill out this paperwork if being seen for headaches or facial pain.

Fill out this paperwork if being seen for problems with memory.

Fill out this paperwork if being seen for Multiple Sclerosis.

Fill out this paperwork if being seen for any other neurological problems.

step 3 - schedule an appointment

Once we have received your completed paperwork, we will contact you to schedule an appointment with Dr. Jetter.
